Patient Rights, Health Records & Complaints
Effective September 1, 2025, under Texas House Bill 4224 (Texas Health &
Safety Code §181.105), Irving Injury Center provides the following instructions
so our patients know how to access their health records, contact our licensing
authority, and file a consumer complaint.
1. How to Request Your Health Care Records
You have the right to request copies of your health care records from Irving
Injury Center. To do so:
- By phone: Call us at (972) 514-6278 to request your records.
- By email: Email your request to clinic@irvinginjurycenter.com.
- In person or by mail: Submit a written request to
Irving Injury Center, 612 N Story Rd #107, Irving, TX 75061. - What to include: Your full name, date of birth, the dates
of service, the records you are requesting, and a way to reach you. - No cost to you: We do not charge our patients a fee to
obtain copies of their own health care records. - Turnaround: Under Texas law, we provide access to your
records within 15 business days of receiving a proper request. Your request
will be handled in accordance with applicable Texas law and federal privacy
regulations (HIPAA).
2. How to Contact Our Licensing / Disciplinary Authority
Irving Injury Center’s chiropractic care is regulated by the Texas Board of
Chiropractic Examiners (TBCE). You may contact the Board at:

3. How to File a Consumer Complaint
If you wish to file a complaint regarding your care, you may do so with the
Texas Board of Chiropractic Examiners. As described under Texas Health &
Safety Code §181.103:
- Online: Submit a complaint through the TBCE complaint
portal at
https://db.tbce.texas.gov/fmi/webd/TBCE_Complaint_Portal?homeurl=https://tbce.texas.gov
or visit the complaint information page at
https://tbce.texas.gov/complaint-information.html. - By phone: Call the TBCE at (512) 305-6700 to request a
complaint form. - By email, fax, or mail: Complaints must be submitted in
writing using the Board’s complaint form, which can be downloaded from the TBCE
website or requested by phone, email, or letter. Mail to: Texas Board of
Chiropractic Examiners, Compliance and Investigations Division, 1801 Congress
Avenue, Suite 10.500, Austin, TX 78701. - Helpful details to include: Provide as much detail as
possible — dates, the name and license number of the chiropractor(s) involved
(if known), the treatment performed, and a description of the issue. Note: the
TBCE does not accept anonymous complaints, and complaints generally must be
filed within six years of the event.
